Patient Documents

For your convenience, patients of California Medical Imaging may print their documents from home and complete them prior to their examination.

New patients must read and sign a COA sheet, and a Patient Registration sheet.

All patients must bring in their photo ID and an insurance card.

For patients undergoing a CT exam or an MRI examination, please fill out the applicable form.
